Job summary

Stockport Homes Group is seeking a CAS3 Support Worker on a part-time basis (18.5 hours per week) with a temporary contract until December 2027. You will support people leaving prison to secure stable accommodation and reintegrate into their communities, using a person-centred approach to set goals and overcome barriers.

Responsibilities

• Move residents into suitable long-term accommodation.
• Complete risk assessments and personalised support plans.
• Deliver regular one-to-one support sessions.
• Work closely with probation services, local authorities and partner agencies.
• Help residents access essentials such as identification, banking, benefits and healthcare.
• Signpost individuals to specialist services including mental health, substance misuse and welfare support.
• Support people to build confidence, independence and positive life outcomes.
