Job summary

Central and North West London NHS Foundation Trust is seeking an Activity coordinator to provide recovery-focused support for individuals with mental health needs. The role is 16 hours per week on a fixed-term basis for 6 months, with face-to-face interviews.

The post involves planning therapeutic activities, working with the MDT, and delivering patient-centred care in hospital, community clinics or home settings as part of CNWL’s multidisciplinary team.

Qualifications

• Good general standard of education (Grade A-C).
• Some experience of IT in a healthcare setting.
• First Aid knowledge.

Responsibilities

• Plan a high-quality programme of therapeutic and occupational activities.
• Deliver ward-based and community activities; work with MDT.
• Assess needs and plan, implement and evaluate individual care plans.
• Deliver direct patient care according to care plans.
