North London NHS Foundation Trust is seeking four Band 6 Community Mental Health Practitioners to join the Intensive Outreach Service, providing recovery-focused interventions in community settings. You will work as a Care Coordinator within a multidisciplinary team, managing a defined caseload and delivering holistic assessments and care planning.

You will undertake biopsychosocial and risk assessments, coordinate services across agencies, and provide intensive outreach to engage service users
