Job summary

East of England Community Health and Care NHS Trust is offering a part-time Community Assistant Practitioner role in the therapy team covering South Norfolk. The post is 22.5 hours weekly across three days, based at Wymondham Health Centre with travel to patient homes and care homes.

You will join a multidisciplinary team, contribute to falls pathways, long-term condition and rehabilitation needs, and support early discharge with mobile, laptop-enabled records.

Qualifications

• NVQ 3 or equivalent knowledge and experience.
• Foundation degree programme or equivalent demonstrable knowledge and skills.
• Good general education and clear written/verbal communication.
• Experience in health or social care settings is essential.
• Experience of lone working and decision making is desirable.
• Experience in End of Life care is desirable.

Responsibilities

• Deliver therapy-related services to patients with long-term conditions in the home and community.
• Work as part of an integrated multidisciplinary team to support early discharge and prevent hospital admission.
• Coordinate with GPs, social services and other agencies to ensure seamless care.
• Maintain patient records and use electronic systems to record activity.
