Job summary

Norfolk Community Health and Care NHS Trust seeks a registered Community Nursing Associate to join the Ketts Community Nursing Team in Wymondham/Attleborough. The role involves visiting patients at homes and care homes, delivering care packages and coordinating with other health and social care professionals to support long-term conditions and rehabilitation.

Qualifications

Registered Community Nursing Associate with current registration.
Excellent communication and IT skills are required.
Experience with assessments and care planning.
Ability to work in home settings and collaborate with multi-disciplinary teams.

Responsibilities

Contribute to delivery of care packages for patients with long-term conditions, palliative care and rehabilitation to promote quality of life and independence.
Assist in facilitating early discharge from hospital and preventing unnecessary readmission.
Coordinate with GPs, social services, inpatient teams and other health and social care professionals to provide integrated care.
Maintain patient records and document visits using the Trust's systems.
