Job summary

Norfolk Community Health and Care NHS Trust seeks a registered Community Nursing Associate to join the Ketts Community Nursing Team in Wymondham/Attleborough. Qualifications

  • Registered Community Nursing Associate with current registration.
  • Excellent communication and IT skills are required.
  • Experience with assessments and care planning.
  • Ability to work in home settings and collaborate with multi-disciplinary teams.

Responsibilities

  • Contribute to delivery of care packages for patients with long-term conditions, palliative care and rehabilitation to promote quality of life and independence.
  • Assist in facilitating early discharge from hospital and preventing unnecessary readmission.
  • Coordinate with GPs, social services, inpatient teams and other health and social care professionals to provide integrated care.
  • Maintain patient records and document visits using the Trust's systems.
