Job summary

Bradcare is seeking a Registered Nurse (RGN) for a part-time role in Swadlincote. You will provide clinical leadership in a nurse-led community environment, delivering complex care safely and in accordance with care plans and guidelines.

You will work within a supportive team, liaising with service users, families and other healthcare professionals to promote independence and quality of life. AUK driving licence and NMC registration are required.

Qualifications

• UK driving licence required.
• Experience or knowledge of TDDI.
• Excellent communication and organisational skills.
• Qualified nurse registered with the NMC.

Responsibilities

• Provide professional nursing expertise and clinical oversight for service users with complex healthcare needs in community settings.
• Support assessment, planning, delivery and review of complex care packages.
• Support safe and effective care delivery in the community leveraging TDDI and complex care knowledge.
• Assist with care-plan implementation, risk assessments and clinical documentation in line with GDPR and professional standards.
• Support safeguarding processes and escalate concerns appropriately.
• Monitor health and wellbeing of service users and escalate as needed.
• Collaborate with GPs, OT’s, community nurses, therapists and hospitals.
