Job summary

Pennine Care NHS Foundation Trust is recruiting a part-time Community Mental Health Practitioner for the CMHT based at Horton House, Hamilton Street Oldham. The post requires a registered and experienced Social Worker to deliver comprehensive mental health care in the community.

You will work with adults of working age, promote an integrated service approach, develop relapse prevention plans and engage with service users and carers within an MDT framework.

Qualifications

• Registered and experienced Social Worker
• Experience in community mental health settings
• Ability to work within a multidisciplinary team (MDT)
• Engagement with difficult-to-engage service users and families

Responsibilities

• Provide care coordination for adults with moderate to severe mental health needs
• Develop relapse recognition and prevention plans and share with MDT
• Engage effectively with service users and families and manage a defined caseload
