Job summary

Pennine Care NHS Foundation Trust is recruiting a part-time Community Mental Health Practitioner for the CMHT based at Horton House, Hamilton Street Oldham. The post requires a registered and experienced Social Worker to deliver comprehensive mental health care in the community.

You will work with adults of working age, promote an integrated service approach, develop relapse prevention plans and engage with service users and carers within an MDT framework.

Qualifications

• Registered and experienced Social Worker
• Experience in community mental health settings
• Ability to work within a multidisciplinary team (MDT)

Job description

We currently have a vacancy for a part time Community Mental Health Practitioner within our CMHT.

The post is based at Horton House, Hamilton Street Oldham

You will need to be a registered and experienced Social Worker to apply for this post.

The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will also work collaboratively across a range of disciplines to continuously promote a 'needs based' approach to all service users.

The practitioner will also promote an integrated approach to service delivery and develop a clear relapse recognition / prevention plans and share with MDT, service users and carers' where appropriate.

The practitioner will develop meaningful engagement with individuals who are often referred to as 'difficult to engage' and their families., The successful applicant will work effectively with adult service users with mental health needs across the CMHT, predominantly in the MDT covering the care coordinator function and will manage the care of a defined caseload of patients.
