Job summary

Pennine Care NHS Foundation Trust is recruiting a Community Mental Health Nurse (Care Co-ordinator) in Oldham. The role involves holistic mental health assessments, risk management, and delivering person-centred care within a Specialist Mental Health Team.

You will work with the MDT to plan, implement, and review care in line with recovery models. The post is Band 6, fixed term for 9 months, full-time with flexible working and compressed hours.

Qualifications

• Registered mental health nurse with current NMC registration.
• Experience in planning and delivering holistic mental health care.
• Ability to undertake risk assessment and care planning.

Responsibilities

• Mental health assessments and risk assessment/management.
• Develop and monitor care plans with MDT input.
• Provide evidence-based therapeutic interventions.
• Educate patients and carers, offering emotional support.
• Promote service-user involvement and recovery-focused care.
• Maintain accurate records and ensure safeguarding compliance.
• Lead and support junior staff within the team.
• Coordinate care within the Community Mental Health Team.
