Job summary

Haydock Medical Centre is seeking a Clinical Pharmacist part-time to focus on medicines optimisation and patient-facing pharmaceutical care across primary care. The role requires GPhC registration with no practice restrictions and an independent prescriber qualification or willingness to work towards one.

You will work with a multidisciplinary team, participate in reviews, and contribute to service development and strategic priorities within the Modality Partnership network.

Qualifications

• MPharm or equivalent recognised pharmacy qualification.
• Postgraduate clinical pharmacy qualification (Clinical Diploma or equivalent).
• Independent Prescriber qualification or willingness to work towards it.
• Membership or Fellowship of the Royal Pharmaceutical Society.

Responsibilities

• Clinically assessing, diagnosing and prescribing where qualified.
• Optimising medicines and improving prescribing safety.
• Supporting medication reviews and structured medicines optimisation.
• Delivering patient-centred pharmaceutical care for long-term conditions.
• Collaborating with a multidisciplinary team to improve patient outcomes.
• Contributing to service development and innovative working practices.
• Working with divisional boards on strategic priorities.
