The Methodist Church in Britain is seeking a two-year fixed-term clinical pharmacist to work across the Northolt PCN in general practice settings. The role focuses on medicines optimisation, including reviews, clinics, and clinician support, with eligibility for independent prescriber and GPhC registration.

Responsibilities include medication reviews, reconciliations on discharge, advising on prescriptions, and contributing to service improvements and education within the primary care team.

Qualificações

• GPhC Independent Prescriber qualification
• Medicines management qualification
• Knowledge of clinical pharmacy practice
• Good written and verbal communication
• Ability to interpret information and give advice
• Problem solving and analytical skills
• Keyboard skills and routine office software such as Word, Excel and PowerPoint
• Awareness of patient confidentiality and safeguarding

Responsabilidades

• Conduct structured medication reviews and long-term condition clinics for patients needing medicines optimisation
• Advise clinicians and patients on medicines with follow-up to monitor changes
• Reconcile medicines on hospital discharge and during transitions to care homes
• Oversee repeat prescribing and electronic prescribing systems
• Provide support for vulnerable patients at risk of admission or readmission
• Identify high-risk cohorts via practice data searches
• Undertake simple prescribing audits and implement changes with the team
• Implement changes from MHRA alerts and local/national guidance
• Support quality initiatives and CQC compliance where medicines are involved
• Educate and train primary care teams and trainees
• Contribute pharmacological advice to new services and analyse medicines data
• Support public health and vaccination campaigns and refer patients appropriately
