Responsibilities

• Undertake initial assessments for new service users, ensuring person-centred care.
• Report to the care manager any issues concerning the care.
• Monitor day-to-day work standards of a team of care workers.
• Maintain records in written or computerised format per procedures.
• Deputise in the absence of the care coordinator and manager.
• Provide cover in other areas as required.
• Participate in back-up on-call rota to deliver care in emergencies.
