Job summary

The Methodist Church in Britain seeks an Advanced Clinical Practitioner for a 10-month fixed-term post at Cleckheaton Group Practice, a town centre clinic offering specialist services. Qualifications

• Experience in a primary care environment.
• Experience in chronic disease management.
• Experience in prescribing and undertaking medication reviews.

Responsibilities

• Manage a caseload across a wide range of presenting problems and needs.
• Make autonomous clinical decisions in consultations, diagnosing, treating or referring patients.
• Diagnose and manage acute and chronic conditions, combining drug and non-drug treatments.
• Prescribe and review medication for effectiveness within scope of practice.
• Make direct referrals to primary, secondary and social services within agreed pathways.
• Ensure care plans are in place, including emergency health care plans.
