Job summary

Dorothy House Hospice Care in Wiltshire is seeking a part-time Exercise Rehabilitation Practitioner to deliver specialist exercise and rehabilitation support to patients and carers.

You will assess needs, develop personalised programmes and work with the AHP team to help people maximise independence and quality of life in a compassionate, evidence-based approach.

The role is based at our hospice and involves 22.5 hours per week over 3 days, with excellent training and development opportunities.

Qualifications

• Excellent communication and interpersonal skills indispensable.
• Organisational skills with the ability to prioritise workloads.
• Registration with a relevant professional body as required.
• Experience delivering education, training or group facilitation.
• Experience working with people with long-term health conditions.

Responsibilities

• Assess patients and develop personalised rehabilitation and exercise programmes.
• Deliver evidence-based exercise interventions to support independence and quality of life.
• Collaborate with Physiotherapists, Occupational Therapists and Clinical Support staff.
• Support patients, families and carers through practical and emotional care.
• Manage referrals and aid delivery of rehabilitative services.
• Contribute to education, self-management and exercise programmes for patients and carers.
• Support students and volunteers and help with their development.
• Maintain accurate records and governance standards.
