Job summary

Phyllis Tuckwell Hospice is seeking a Living Well Registered Nurse to join the Community Palliative Care Team in Guildford. The role emphasizes holistic assessment, care planning, and delivering patient- and family-centred care across home, outpatient, and community settings.

Responsibilities

• Assess, plan, implement and evaluate patient care plans with patients and carers.

• Deliver care in patients' homes, outpatient, or community settings and ensure proper documentation.
