St Elizabeth Hospice in Ipswich area is seeking a part-time Community Care Coordinator to coordinate patient services in the palliative care hub. You will be the first contact for the 24/7 OneCall line, supporting a mix of home, community, and hospice settings and working shifts across 7 days.
