Job summary

St Elizabeth Hospice in England seeks a part-time Community Care Coordinator to join our palliative care coordination hub. You will be the first contact for the OneCall 24/7 advice line and coordinate patient services across our community and clinical teams, supporting referrals and appointment allocation.

The role covers a mix of 6 hour and 7.5 hour shifts, 7 days a week from 8am to 8pm, with full training and a strong focus on safeguarding and compassionate care.

Qualifications

• Experience in busy administration roles with minimal supervision.
• Experience establishing work processes and procedures.
• Experience in healthcare or safeguarding contexts is desirable.
• GCSEs or equivalent; NVQ Level 3 or higher in Business Administration.

Responsibilities

• Be the first point of contact for callers to the OneCall 24/7 advice line, directing to the appropriate service.
• Respond to all calls in a professional, calm and empathetic manner.
• Receive and process new referrals, liaising with clinicians and patients to ensure information accuracy.
• Coordinate visits by Community Healthcare Assistants and allocate new appointments with clinicians.
• Organise care packages for end-of-life patients and maintain patient records with timely updates.
• Provide administrative support for community, medical, therapy, emotional and safeguarding teams.

Education

GCSE Standard or equivalent

NVQ Level 3 or above in Business Administration
