Job summary

Marie Curie is seeking a Clinical Nurse Specialist to join a new service across South Liverpool PCN, working with GP practices and community hubs to support palliative and end-of-life care. You will manage a caseload, provide specialist advice, and coordinate care across primary and community settings.

You will be autonomous, undertake complex conversations, support advance care planning, and advocate for person-centred care across organisations.

Qualifications

• Registered Adult Nurse with current NMC registration.
• Nursing degree and Level 6 post-registration qualification.
• Experience in specialist palliative care.
• Ability to assess complex palliative needs and coordinate care across services.

Responsibilities

• Work across GP practices, community hubs and other locations to proactively support palliative and end-of-life care.
• Manage a caseload and coordinate care across organisations.
• Provide expert palliative-care advice and participate in advance care planning.
• Outreach and referrals, building connections across services.
