Job summary

City Health Care Partnership CIC in Driffield is seeking a Macmillan Clinical Nurse Specialist – Palliative Care to lead expert assessment and support for patients, families and professionals. You will work independently and with colleagues to deliver compassionate, evidence-based care and contribute to service development.

The post requires a Registered Nurse with current registration, strong clinical and communication skills, and experience in community nursing and multidisciplinary teams.

Qualifications

• A Registered Nurse qualification and current professional registration.
• Knowledge of community nursing, patient assessment and evidence-based practice.
• Strong communication skills and the ability to build positive relationships with patients, carers and colleagues.
• Experience working in healthcare and as part of a multidisciplinary team, or transferable skills from a similar setting.
• A flexible approach, willingness to learn and commitment to delivering compassionate care.

Responsibilities

• Assess patients’ needs and deliver safe, person-centred nursing care in line with care plans.
• Triage, assess and provide unplanned care to support patients in the community and help prevent hospital admissions.
• Work with multidisciplinary teams and partner organisations to coordinate effective care and referrals.
• Maintain accurate clinical records and contribute to high standards of patient safety and care quality.
• Support learning and development by sharing knowledge with colleagues, students and carers.
