Job summary

City Health Care Partnership CIC in Driffield is recruiting a Macmillan Clinical Nurse Specialist – Palliative Care (Band 7) on a permanent full-time basis (37.5 hours per week). The post offers a salary of £49,387 - £56,515 per year with a 37.5 hour week and a closing date of 6 November 2026.

The CNS will lead expert palliative care, advise and support patients, families and staff, and help develop services.

Qualifications

• Registered General Nurse with current professional registration.
• Knowledge of community nursing, patient assessment and evidence-based practice.
• Experience working in healthcare and as part of a multidisciplinary team; flexible, compassionate care.

Responsibilities

• Assess patients’ needs and deliver safe, person-centred nursing care in line with care plans.
• Triage, assess and provide unplanned care to support patients in the community and help prevent hospital admissions.
• Work with multidisciplinary teams and partner organisations to coordinate effective care and referrals.
• Maintain accurate clinical records and contribute to high standards of patient safety and care quality.
• Support learning and development by sharing knowledge with colleagues, students and carers.
