Job description

You will be based at Livingstone Hospital, Dartford, as part of our community nursing team who provide care to people in their own homes or within a clinic setting.

We empower our staff to work autonomously and flexibly by providing state-of-the-art tablets to check in from home, write notes on the go, and avoid being confined to an office, enabling you to enjoy working in your preferred community environment. You will work as part of the out-of-hours team within the community, delivering healthcare between 8pm - 8am.

As this is a community-based role, you will need to have a UK driving licence and access to a car.

Main duties of the job

Working closely with our Community Nursing Team, as a Healthcare Assistant, you will support senior team members to provide care to people in their own homes or within a clinic setting for a wide range of conditions including diabetes, wound care, and end-of-life care. Our work is crucial to the community as we aim to prevent unnecessary hospital admissions. In this role, you will care for patients in the way you believe is best, delivering high service standards, sharing best practices, and actively improving our ways of working.
