Job summary

We are seeking a dedicated and compassionate Band 5 nurse to join our out of hours community nursing team. Delivering high quality responsive care to patients in their own homes across the Sandwell and West Birmingham area.

Working closely alongside our Urgent response palliative care nurses you will play a vital part in helping patients remain at home , providing care to prevent unplanned hospital admissions.

The successful applicant will have excellent clinical assessment skills , the ability to work under pressure and a commitment to delivering compassionate care.

A flexible, responsive approach and good communication skills are also a essential aspect of this role as you are often the first point of contact for our patients overnight.

Main duties of the job

As an Out of Hours Community Nurse, you will play a vital role in delivering responsive, high-quality care to patients in their own homes overnight. This is a dynamic and varied role, supporting patients with urgent nursing needs across a range of conditions. You will manage pressure areas and wounds, support diabetes care including insulin administration, carry out clinical assessments, administer medication via a variety of routes , and provide personal care where required. You will also complete verification of expected deaths when necessary.

In addition to your independent work, you will work closely alongside the Urgent Response and Palliative Care Team to support patients requiring end-of-life care, ensuring comfort and dignity while helping them to remain at home wherever possible.

You will supervise and delegate to Band 2 Healthcare Assistants, maintaining high standards of care throughout the night. Your role will be key in delivering timely interventions that help patients avoid unnecessary hospital admissions and receive the right care at the right time.
