Job summary

Qualifications

• Significant post-registration experience (minimum 3 years) in acute or primary care.
• Evidence of autonomous practice assessing and managing patients with undifferentiated presentations.
• Leadership skills and first level professional registration.
• Non medical prescribing qualification and MSc Advanced Practice.

Responsibilities

• Provide assessment and management for out-of-hours patients, including home visits and telephone consultations.
• Discharge patients and refer to other professionals as needed.
• Contribute to clinical governance, audit and risk assessment.
• Deliver patient-centred care across all ages with safe, evidence-based practice.
