Job summary

Sussex Community NHS Foundation Trust is seeking an Operational Lead for the UCR Service, delivering a two-hour crisis response seven days a week to prevent unnecessary hospital admissions in the community.

You will lead a multidisciplinary team of GPs, nurses, paramedics, pharmacists, occupational therapists, physiotherapists and support staff; coordinate shifts and visits; and manage performance and a devolved budget to secure safe, high‑quality care at the right time and place.

Qualifications

• Experience leading urgent community care teams.
• Strong clinical governance and supervision skills.
• Ability to manage budgets and KPIs.
• Excellent cross-agency collaboration.

Responsibilities

• Lead the UCR MDT to deliver rapid community care.
• Coordinate shifts and visits across Sussex.
• Support 7-day service delivery 8am–8pm.
• Develop service through reflective practice and quality improvement.
