Job summary

The Pharmacist Network is seeking an onsite Clinical Pharmacist in Salford to support a partner GP practice. The role focuses on safe medicine use, payments and service improvements, within a collaborative team. 30 hours per week, onsite only.

You will work with pharmacists and pharmacy technicians to ensure optimal patient care, manage polypharmacy reviews, and provide expert guidance on medications and related queries.

Qualifications

• Minimum 12 months experience in general practice/primary care.
• Registered Clinical Pharmacist with GPhC registration.
• Experience with EMIS or Systemone (System1).
• Completed the CPPE Primary Care pathway or near completion.

Responsibilities

• Provide pharmaceutical care and advice to patients within primary care.
• Resolve medication-related queries as part of a multidisciplinary team.
• Conduct medicines reconciliation and post-discharge reviews.
• Lead medicines optimisation and quality improvement initiatives.

Clinical Pharmacist IP - Onsite in Salford

The Pharmacist Network are a pharmacist led organisation who provide general practices nationally, with the delivery of pharmacist and pharmacy technician services. There is a wealth of experience and expertise within our current workforce of 250+ Pharmacist and Pharmacy Technicians. Our senior clinical team support the training, development and workflow management of Pharmacists and Pharmacy Technicians within primary care and PCN settings.

We currently have an exciting opportunity for an IP Clinical Pharmacist, to work full time for our partner practice within Salford, this opportunity is available for highly motivated and focused Clinical Pharmacist, who will be responsible for the smooth running of day-to-day activities within general practice, while playing a vital role in providing pharmaceutical care and advice to patients focusing on best practice and adhering to safety guidelines.

This vacancy is for 30 hours a week.

The Role

As an onsite Clinical Pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.

You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.

Main duties of the job

• As a clinical pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's.
  
• Polypharmacy medication reviews and SMRs
  
• Face to Face medication reviews
  
• Help to utilise skill mix
  
• Ensure better access to healthcare
  
• Support practice staff with prescription and medication queries
  
• Medicine reconciliation and review post-discharge
  
• Conduct medical related audits and work on service improvement with the practice
  
• Provide expert advice on medications to other clinicians and patients
  
• Deliver clinical leadership on medicines optimisation and quality improvement
  
• Ensure appropriate and efficient use of medication
  
• Educate patients on medication and disease prevention
  
• An understanding of QOF, CQC and local incentive scheme workstream
  
• Create and follow clinical pharmacy programme according to practice policy
  

Requirements

• A minimum of 12 months experience of working within general practice/primary care
  
• A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
  
• Experience with EMIS or System 1
  
• Completed the CPPE Primary Care pathway or close to completion
  
• Able to commute to onsite location / live within close proximity of practice
  
• Excellent communication and interpersonal skills
