Job summary

NHS Jobs is seeking an onsite Clinical Pharmacist to work closely with a team of pharmacists and pharmacy technicians. You will help resolve medication queries, perform polypharmacy reviews and SMRs, and conduct face-to-face medication reviews to improve patient outcomes.

You will provide expert medication advice, support practice staff, and lead medicines optimisation initiatives. A GP-friendly environment and access to ongoing professional development are offered.

Qualifications

• A minimum of 12 months experience in general practice/primary care.
• GPhC registration with a relevant qualification.
• Independent prescribing qualification.
• Experience with EMIS or System One.
• Completion of the CPPE Primary Care pathway or close to completion.
• Must be able to commute to onsite location.

Responsibilities

• Resolve medication-related queries.
• Polypharmacy reviews and SMRs.
• Face-to-face medication reviews.
• Support practice staff with prescription and medication queries.
• Medicine reconciliation and review post-discharge.
• Conduct medical audits and drive service improvement.
• Provide expert advice on medications to clinicians and patients.
• Deliver leadership on medicines optimisation and quality improvement.
• Educate patients on medications and disease prevention.
• Understand QOF, CQC and local incentive schemes.
