Job summary

Case Management Cymru Ltd. is looking for a warm, patient, and reliable Support Worker to assist a client in Swansea who has an acquired brain injury.

You will build a trusting relationship, provide gentle prompting and reassurance, and help maintain independence, routine and meaningful community engagement. A full induction and training will be provided, and the role is subject to an Enhanced level DBS check.

Qualifications

• Full UK driving licence with access to a suitable vehicle
• Experience of supporting an individual with an acquired brain injury
• Patient, reliable and flexible approach
• Ability to build and maintain a positive professional relationship with the client and her family
• Ability to work collaboratively with the wider team
• Willingness to register with Social Care Wales, if not already registered

Responsibilities

• Build a positive, trusting relationship with the client
• Provide gentle prompting and reassurance
• Support independence, routine and meaningful community engagement
• Work with the client’s family and wider professional team
• Receive ongoing supervision from the Case Manager
