Job summary

Leaders In Care Ltd is recruiting an Occupational Therapist to work in a community-based Adult Social Care team, delivering person-centred rehabilitation to adults in their own homes.

You will complete home-based assessments, develop rehabilitation plans, recommend equipment and minor adaptations, and maintain accurate case records while coordinating with social workers and health professionals.

Qualifications

• Degree in Occupational Therapy (registered or eligible)

The role

You will complete home-based assessments to understand an individual’s needs, risks and rehabilitation potential. Working alongside the person and relevant professionals, you will develop practical rehabilitation plans and goals, recommend suitable equipment, minor adaptations and daily living techniques, and review progress throughout their support. You will also maintain accurate case records, contribute to case triage and offer Occupational Therapy guidance to colleagues.

What you’ll need

• Degree in Occupational Therapy, or due to qualify shortly
• Experience of working with vulnerable adults and supporting daily living activities
• A strong understanding of rehabilitation and how Occupational Therapy improves independence
