Job summary

Qualifications

• PVG Scheme membership is essential on taking up appointment.

Responsibilities

• Conduct functional assessments to determine care needs.
• Provide equipment and minor home adaptations.
• Support goal setting and deliver tailored interventions to maximise independence.
• Manage a varied caseload and collaborate with Social Care Workers, Home Carers and multidisciplinary teams.
