Job summary

Service Care Solutions are recruiting an experienced Occupational Therapist to join the Integrated Neighbourhood Team (INT) in Manchester. You will assess individuals, identify barriers to independence and deliver person-centred outcomes in collaboration with social workers and health professionals.

The role focuses on prevention, reablement and rehabilitation, helping people stay safely in their homes while reducing demand on formal care services. Hybrid working is available.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Experience in community-based occupational therapy.
• Skilled in functional and moving/handling assessments.
• Experience recommending equipment and home adaptations for independence.
• Knowledge of Care Act 2014 and adult social care.
• Strong communication and multidisciplinary teamwork.
• Full UK driving licence and access to a vehicle.

Responsibilities

• Undertake comprehensive functional assessments to identify needs and risks.
• Carry out moving and handling assessments.
• Assess and recommend equipment and adaptations to promote independence.
• Support individuals to remain safely at home and in the community.
• Work with social workers and other professionals to develop support plans.
• Provide specialist OT advice to individuals, carers and partner agencies.
• Contribute to safeguarding and intervention processes.
• Promote strengths-based practice, prevention and rehabilitation.
• Support hospital admission avoidance and reduce residential care.
• Maintain accurate assessments and documentation.
