Sanctuary Personnel is recruiting an Occupational Therapist for the Double Handed Care Project in Hackney, London. This full-time role is a 12-month, ongoing assignment delivering expert OT services and home visits in the community, aiming to reduce double-handed care while improving safety and patient independence.

You will conduct comprehensive assessments, develop bespoke care plans, train care staff, and maintain precise clinical documentation in line with HCPC standards.

Qualifikationen

• HCPC registration is required and must be current.
• Experience in Adult Social Care settings, focusing on moving and handling.
• Proven track record in managing complex moving and handling cases.
• Strong assessment, clinical reasoning, and report-writing skills.
• Ability to develop and review care plans and train care staff.

Aufgaben

• Deliver specialist Occupational Therapy services within the Double Handed Care Project.
• Complete comprehensive Occupational Therapy assessments and evaluate existing care arrangements.
• Develop bespoke care plans aimed at maximising patient independence and increasing safety.
• Identify opportunities to reduce or eliminate the need for double-handed care when appropriate.
• Conduct moving and handling assessments and provide training for care staff.
• Review and evaluate care plans regularly to ensure continued suitability.
• Undertake community visits and reassessments to support patient care delivery.
• Maintain accurate, up-to-date records, reports, and clinical documentation with precision.
