Key Responsibilities

• Conduct comprehensive functional assessments of patients in their home or community environments.
• Develop and implement personalised rehabilitation plans, focusing on mobility, self‑care and daily living skills.
• Provide equipment and adaptations recommendations to support safe and independent living.
• Work as part of a multidisciplinary team (MDT) including physiotherapists, social workers and community nurses.
• Support hospital discharge planning and facilitate safe transitions into community settings.
• Monitor and evaluate patient progress, adjusting care plans as required.
• Maintain accurate and timely patient documentation in accordance with professional standards.
• Contribute to service development, supporting the delivery of high‑quality occupational therapy interventions.
• Educate patients, carers and family members on rehabilitation techniques and safe equipment use.
• Participate in case discussions, clinical supervision and continuous professional development (CPD) activities.
