• Experience assessing home environments and recommending adaptations.

Responsabilidades

• Assess new referrals to the service and identify needs relating to access, safety, and independence in the home environment.
• Recommend and coordinate necessary adaptations and equipment based on assessments.
• Support junior staff within the SSOT section to enhance team performance.
• Complete onward referrals as required to ensure service users receive the required support.
• Produce comprehensive reports and attend the Major Adaptations Panel as needed.
