The Reablementteam assists patients requiring additional support and rehabilitation ondischarge from hospital, and those in the community who need increased care ortherapy input. Assessments are holistic and focused on maximising independenceat every stage. Patients are at the heart of everything we do.
This post covers Redditch and Bromsgrove, with a 5 shift weekly pattern. Parttime and Flexible working considered, rising to a 9 month contract offer with30 hours over 4 shifts.
As a memberof the reablement team, you will work closely with therapists and social careteams, having the unique opportunity to experience both sectors whilst with us.
You willperform holistic, goal focused, patient centred assessments, and developthorough treatment programmes to meet the goals. You will manage a variedcaseload, spanning all Occupational Therapy specialities.
You will workclosely with both health and social care colleagues, in a blended approach topatient care. Physiotherapists, nurses, occupational therapists, therapyassistants and social care staff, undertake collaborative working on a dailybasis to ensure that all patient needs are met. Regular clinical supervisionand professional support are provided. We positively encourage any specialinterests which complement the patient demographic that we see.
