Job summary

North Cheshire and Mersey NHS Foundation Trust in Halton is recruiting a Band 2 Nursing Assistant to support the Community Winter Stepdown Ward on Ward B3 from 1 December 2026 to 31 March 2027. Qualifications

• Experience in a care setting is desirable.
• Ability to assist with activities of daily living and mobility.
• Willingness to work as part of a multidisciplinary team.

Responsibilities

• Deliver high-quality care to medically optimised patients requiring rehabilitation and discharge pathway support.
• Assist patients with personal care activities while promoting dignity and independence.
• Encourage patients to maximise functional abilities in line with rehabilitation goals.
• Support patients to engage in activities to promote recovery and discharge planning.
• Collaborate with nursing, therapy and discharge planning teams to support timely discharge and patient flow.
• Monitor and report changes in patient condition, mobility, cognition or wellbeing to registered staff promptly.
• Undertake delegated clinical observations and patient monitoring activities.
• Assist with mobility, transfers and safe use of equipment per moving and handling guidance.
• Support patients during mealtimes and document nutritional/hydration needs.
• Maintain accurate patient records in line with Trust policies.
