A regional healthcare provider is seeking a Nurse Led Clinic Practitioner to assess and treat patients with serious mental health difficulties. Responsibilities include managing patient caseloads, providing support to junior staff, and working collaboratively within a multidisciplinary team. The post holder will work as a member of a small, dedicated clinic providing assessment and treatment to patients experiencing mental health difficulties of a serious and enduring nature.
To be responsible for a caseload of patients who require assessment and monitoring as part of their ongoing treatment within secondary mental health services.
To provide support and guidance to junior staff members working into the Clozapine and Lithium Clinic where required.
To work as part of a wider system of care provision to ensure patient journey is as seamless as possible.
To provide professional advice and support to other agencies and members of the multidisciplinary team.
