Mid Cheshire Hospitals Foundation Trust is seeking a Staff Nurse (Band 5) for the Frail Elderly Assessment Unit at Leighton Hospital, Crewe. Role is 37.5 hours per week with a mix of days and nights across seven days, on a 9-month secondment. Salary £32,073 - £39,043 per annum pro rata.

You will contribute to comprehensive geriatric assessments and rapid discharge planning. The successful candidate will lead care planning, support and supervise the team, and work closely with multidisciplinary

Formación

• Qualified Nurse with current NMC registration.
• Experience in care planning for frail elderly patients.
• Strong communication and leadership abilities.
• Ability to work across multidisciplinary teams.
