Job summary

Northern Care Alliance NHS Foundation Trust in Heywood, Middleton, and Rochdale area is hiring Staff Nurses (Band 5) for integrated neighbourhood teams. Open to internal candidates only initially.

You will assess nursing needs, develop and evaluate care plans, and provide high-quality home-based care, including palliative and wound care. Travel across the Trust footprint is expected; post-registration experience and preceptorship are required.

Qualifications

• Must be registered with NMC and have post-registration experience.
• Completed registered nursing preceptorship is required.
• Evidence of continued professional development and good communication skills.

Responsibilities

• Assess nursing needs and develop personalised home care plans.
• Deliver high-quality, holistic community care in patients' homes.
• Evaluate outcomes to ensure safe, effective care.
• Support end-of-life care and wound/palliative care interventions.
