Nurse Case Manager

£45,000 per annum

Monday to Friday, 9am to 5pm

Role:

As a Nurse Case Manager (Tracheostomy Experienced), you would be responsible for providing face-to-face support to our clients and their families while creating personalised care plans for each client. You would work closely with a core team of carers, training and supporting them with clinical skills and activities.

Areas:

North East London

Key Requirements:

• Registered Nurse with a minimum of 12 months' work experience in the UK and an active NMC PIN
• Tracheostomy and ventilator experience desirable
• ITU/HDU experience would be desirable
• Knowledge of care for spinal patients desirable
• Ability to work flexibly and respond quickly to changing priorities while meeting deadlines
• Ability to build strong relationships with clients, their families, and carers
• Supportive management style that can bring out the best in others
• Confident, passionate, and possess a "can-do" attitude
• Ability to manage your own caseload of community packages, including working in the community and at Wimbledon head office
