Overview

We are seeking a passionate Nurse Case Manager to join a well-established provider specializing in adult complex care within the community. This role is based in the Hounslow area, with an attractive salary of up to £45,000 per annum. The successful candidate will support adult clients and their families by creating personalized care plans and working closely with a team of carers.

Criteria for Nurse Case Manager

• RGN with an active NMC PIN
• Supportive management style that encourages team development
• Recent experience (within 2 years) in tracheostomy care and ventilator management
• Excellent verbal and written communication skills for effective liaising with patients, families, and multidisciplinary teams
• Suitable for Band 5 / Band 6 Nurses with leadership experience managing HCAs
• Full UK Driving Licence (desirable but not essential)

Key Responsibilities

• Design and implement tailored care plans for individual clients
• Manage the multidisciplinary team through effective communication and teamwork
• Monitor, support, and assess clients’ physical and physiological needs
• Deliver nursing interventions, including medication management
• Build and sustain strong relationships with clients’ families
• Collaborate with community teams, CHC nurses, and CCG commissioners to establish bespoke care packages
