The RN Case Manager monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.

Responsibilities include:

1. Communicating with patients, families, nursing staff, physicians, and other healthcare team members to ensure proper case management and effective discharge planning.
2. Monitoring patient and family satisfaction, responding to questions and complaints regarding care.
3. Participating in discharge planning, including coordinating patient transfers and community resource referrals, and providing discharge education.

Fauquier Health is a 97-bed acute care hospital offering surgical services (including robotics), a 24-hour Emergency Department, extensive medical imaging, an Intensive Care Unit, and more. We provide a variety of specialty services including Orthopedics, Spine, Cardiac Catheterization, and robotic-assisted surgery. Our Family Birthing Center and Intermediate Care Nursery are located on the fourth floor.

Qualifications

• Minimum Education: Bachelors Degree in Nursing (BSN) preferred
• Required Skills: Minimum 15 CEUs per year
• Licenses: Current Virginia RN license
• Certification: Basic Life Support (BLS), obtainable during first week of orientation
• Minimum Work Experience: 3 years of nursing experience required; experience in utilization review, case management, discharge planning, and clinical leadership preferred
