Job summary

Mid Cheshire Hospitals NHS Foundation Trust is seeking a Nurse Associate or Assistant Practitioner to join our community care team in Alsager. You will assess, deliver and evaluate nursing care under the guidance of a case manager/District Nurse, supporting patients with complex health needs in their homes and local clinics.

You will work with a multidisciplinary team to reduce hospital admissions, develop skills, and be mentored to achieve competencies within your scope of practice.

Qualifications

• Qualified AP or NA with foundation degree or equivalent.
• Experience in community health or district nursing preferred.
• Good communication and interpersonal skills.

Responsibilities

• Work under supervision of a Registered Nurse/Practitioner in community settings.
• Assist in assessment, delivery and evaluation of nursing care.
• Develop clinical skills and support patients to live independently.
