Job summary

SMASH Care Community in Alsager is seeking a Nurse Associate or Assistant Practitioner to join our community care team. You will assess, deliver and evaluate nursing care under the guidance of a case manager or District Nurse, supporting patients with diverse health needs in their homes and community settings.

You will work with patients and families to empower self-care and independence, collaborating with multi-disciplinary colleagues to minimise hospital admissions.

Qualifications

• Effective communication and interpersonal skills required.
• Adaptable, reliable and approachable in community care settings.
• Mentoring and supervision will be provided to develop competencies.

Responsibilities

• Assess, deliver and evaluate nursing care under guidance of a case manager/District Nurse.
