We are seeking a motivated and compassionate NurseAssociate to join our friendly and progressive teamat ChurchLane Surgery. This is a fantastic opportunity to be part of amultidisciplinary team delivering personalised, high-quality care within theheart of the local community.
The Nurse Associate will support the clinicalteam in delivering care across a wide range of services within the GPpractice/PCN. Under the supervision of registered nurses and GPs, you will playa key role in health promotion, disease prevention, and deliveringpatient-centred care.

We are a rural practice, withover 12,300 patients, working from a modern purpose-built premises.
We are also a training practicewith a wide multidisciplinary team including first contact physiotherapists, aPCN pharmacy team, Health & Wellbeing Coach, Social Prescriber, MentalHealth Practitioner, and an in-house dispensary.
This is a 12-months fixed term position, 29-35hours per week, with as soon as possible/immediate start date.
Responsibilities: This isnot an exhaustive list
Support chronic diseasemanagement clinics e.g. asthma, diabetes, hypertension
Performclinical observations, wound care, phlebotomy, ECGs and health checks
Administervaccinations and immunisations in line with national guidance
Supportcervical screening
Promoteself-care and healthy lifestyle choices through patient education
Maintainaccurate, timely documentation and contribute to care planning
Participatein quality improvement and population health initiatives across the PCN
Work collaboratively with the wider MDTto ensure high standards of care
Cervical Screening (if trained)
