The postholder will have successfully completed the 2-year Trainee Nursing Associatetraining programme, which includes the attainment of a level 5 FoundationDegree equivalent qualification and NMC registration.

7.Report to appropriate registered careprofessionals information received from individuals, carers and team membersto ensure the best possible care and to continue to promote multi-disciplinaryworking/partnership working.

ClinicalResponsibilities

9.Deliver high quality, compassionate care under thedirection of a registered Nurse (or other registered professional dependent onthe setting) with a focus on promoting health and independence.

10.Be responsible and accountable for own practice,working within limits of competence and within professional boundaries whileraising concerns to a registered care professional or appropriate person as andwhen required.

11.Carry out specific delegated clinical tasks andresponsibilities to a high standard and competency which promotes excellent patient care and continued partnership working.

12.Where appropriate work without direct supervision,at times delivering care independently in line with the individuals plan ofcare, within designated parameters of practice of the Nursing associate role,accessing clinical and care advice when needed while providing feedback onprogress against the care plan.

13.Maintain understanding of all elements of thenursing process

14.Support individuals with all aspects of careincluding daily living, providing person-centred care and promoting health andindependence i.e. Making Every Contact Count.

15.Manage and monitor the condition and health needsof individuals and their families

16.Undertake tasks that are relevant to the specialityincluding health assesments as per trust policy.

17.Accurately record nutritional and fluid intake toidentify signs of dehydration or fluid retention and elevate as necessary.

18.Use contemporary assessment tools e.g. ASQs, SDQsand nutritional assessments, emotional well-being tools

19.Manage and monitor effectiveness of symptom reliefmedication, seeking the advice of a trained member of staff when required.

20.Assist in the implementation of appropriate actionto meet the specific physical, emotional, and psychological, social, culturaland spiritual needs of individuals, families and carers.

21.Assist the Registered Nurse with complex care asdeemed appropriate.

22.Maintain an understanding of caring for individualswith particular conditions for examplemental illness, learning disabilities.

23.Ensure that privacy, dignity and safety ofindividuals is maintained at all times.

24.Ensure that all precautions are taken to safeguardpatients property and valuables.

25.Recognise issues relating to safeguardingvulnerable children and adults and report any problems or raise concerns to theappropriate registered care professionals.

26.Assist with the implementation and monitoring ofclinical standards and outcomes.

27.Demonstrate a good understanding of the MentalCapacity act / Deprivation of Liberties and applies principles to everydaypractice seeking advice / guidance from Registered Nurse or other registeredprofessional as required.

28.Demonstrate good understanding of principles ofconsent and ensure valid consent is obtained prior to undertaking nursing andcare procedures.

29.Deliver effective care following plans determinedby the registered nurse or other registered care professional and providefeedback on progress against the plan.
