Responsibilities

• Monitor each resident's wellbeing and update care plans as needed.
• Participate in person-centred care planning and monthly evaluations.
• Support residents with medication requirements.
• Undertake staff supervision and support development.
• Act as a key point of contact with residents, relatives and professionals, addressing requests and escalations.
