We are looking for a confident Night Support Worker with a passion for delivering outstanding care. You will work 42 hours a week at Cygnet Storthfield House, a 22-bed specialist high support inpatient rehabilitation (level 2) service for men in Derbyshire, dedicated to providing a high-quality environment for promoting long-term recovery.
From the moment individuals arrive, you will work with them to maximize positive outcomes and recovery, focusing on discharge plans to enable them to move to the least restrictive environment suitable for them. For some, this could be independent living; for others needing additional support, we provide a step-down service, 15 The Sycamores, a 4-bed facility offering continued support as part of their recovery journey, supported by the multidisciplinary team (MDT).
By combining a collaborative, multidisciplinary approach, we deliver high-quality care using our unique 4-stage model of care for our mental health high support inpatient rehabilitation (level 2) services.
