Job summary

HCRG Care Group in Swindon is seeking an Out-of-Hours Community Health Navigator to join the night shift team. You will be the friendly first point of contact for patients, visitors and professionals, ensuring timely access to the right support and accurate data entry into clinical systems.

You will assess urgency, coordinate with hospital and community services, and assist with safety monitoring while working as part of a supportive team.

Qualifications

• Excellent communication and customer service skills.
• Experience handling enquiries and coordinating care.
• Ability to enter accurate patient information into systems.

Responsibilities

• Act as first point of contact for incoming calls.
• Manage enquiries and enter patient data into clinical systems.
• Assess urgency and respond promptly.
• Handle Major Incident alerts and record actions taken.
• Coordinate with external partners for seamless care.
• Monitor fire alarms, CCTV and security systems.
