Job summary

HCRG Care Group in Chippenham is hiring a Community Health Navigator to be the first point of contact for patients, carers and professionals, guiding them to the right community health services. The role supports enquiries by phone, email and post, handles referrals, and maintains confidential records with compassion and professionalism.

The position offers either part-time or full-time hours (22.5–37.5 per week), a daytime service across 7 days, and on-site training with potential days from

Qualifications

• GCSE Maths and English (Grade A-C/Level 4 or above) or equivalent qualification.
• Previous administration experience in a busy customer-facing environment.
• Experience using a range of IT systems and databases.
• Strong computer literacy, including Outlook, Word and Excel.
• Accurate data entry and keyboard skills with attention to detail.
• Excellent verbal and written communication skills.

Responsibilities

• Be the first point of contact for patients, carers, healthcare professionals and partner organisations.
• Provide information, advice and signposting to appropriate services.
• Process incoming referrals and direct to the correct team.
• Maintain patient records and manage referrals with confidentiality.
• Coordinate appointments and clinical schedules within agreed timescales.
