Choice Care is recruiting a Night Social Care Worker to support adults with learning disabilities, autism and mental health needs within our Derby area homes. You will provide compassionate personal care, assist with daily activities and follow tailored care plans to promote independence and wellbeing.

Responsibilities

• Assisting those we care for with personal care needs.
• Medication administration.
• Assisting and engaging in daily activities, hobbies or outings.
• Following care plans in line with policies and procedures.

• Maintaining written and electronic daily records.
• Attending and completing required training.
